Tracy Sacré is a passionate entrepreneur and visionary in the home care sector. As a former Intensive Care Unit nurse and mother of two, Tracy has an intimate knowledge and understanding of how to care for those who, through the course of time or events, are unable to care for themselves. As an Instructor of Nursing at Vancouver Community College, she has an extensive knowledge about the latest research and advancements in nursing. Tracy’s RN background as well as her time working in Neurology and Stroke Research gives her valuable insight in to what might be happening to our loved ones. Tracy sits on the Board of Directors for the North Shore Keep Well Society. North Shore Keep Well Society’s purpose is to promote the health and well being of older adults living on the North Shore. To encourage seniors to help themselves and each other, to live in better health, to learn, and to expand their experiences of well-being. Keep Well’s purpose aligns with Tracy’s personal and professional goals, values and mission. Rather than doing business simply for profit, Tracy’s legacy is to change the way we care for our elderly, to add value to those who gave us values. Tracy’s warm, welcoming, approachable personality puts anyone who meets her instantly at ease…not to mention the way her laugh fills a room. Tracy was born and raised in North Vancouver, where she currently lives with her husband and two daughters.

Bringing a wealth of big and small business experience, Sam is an inspirational leader and a service innovator that has a history of setting the highest standards in multiple industries. With decades of experience, Sam is a sought-out leader in Vancouver’s business community. Sam joined Proof of Care in 2016. In his role as General Manager, Sam oversees day-to-day operations, including management of Proof of Care’s leadership team and more than 70 employees, as well as the rollout of a series of exciting new enterprises that are well underway. Through his years of business service, Sam has gained proven leadership expertise in operations, sales, and staff development, making him a valuable asset to Proof of Care. Sam also sits a Vice-Chair on the Vancouver Senior Service Providers Board of Directors.

Maryam is an exceptional Nurse with a career built on supporting senior's health here in Vancouver. Graduating with honours from CDI College, Maryam is a professional Nurse and passionate advocate for Senior's care. Her compassionate, organized and detail-oriented care style has long made her a favourite of our clients at Proof of Care.  

Maryam has been a key staff member of Proof of Care since 2016. She has been promoted twice in her career with POC and now manages our Nursing and Caregiver teams and is responsible for all of our care services.

MARYAM NAVEED

Simone graduated the University of Western Sydney in 2012 as a Registered Nurse (RN) before gaining a wealth of experience in a broad range of care in hospitals in Australia. Drawn to Vancouver's natural beauty, Simone moved to Canada in 2019 and joined Proof of Care's prestigious management team as Care Manager. Simone oversees the health and wellbeing of all of our clients and acts as the key contact to our clients and their loved ones.

Raman is a dedicated, energetic and professional Licensed Practical Nurse (LPN) who leads our amazing career caregiving staff to be the best trained and motivated team in-home care staff in Canada. Raman also ensures that our clients are always getting a little extra attention from all of us at POC, through fantastic outings, games, movie nights and even virtual reality experiences. Raman is the key mentor to our staff and leads our world-class training programs.

After meeting with the Proof of Care team, it was clear that Claudia had similar values and vision for what quality care should be. Claudia’s passion for exceptional healthcare makes her the ideal person to manage our complex scheduling needs and to support our clients and staff with her wealth of knowledge and endless enthusiasm. A student of CDI College, Claudia had exceeded as a caregiver and medical services coordinator prior to joining our wonderful team at Proof of Care. Claudia is focused on providing the best possible experiences for not only our clients but also for our world-class staff.
